Funeral Benefit Nomination Form

How to use this nomination form:

  • You nominate two people on the Funeral Benefit Nomination Form and send it to your HRA.
  • One of these two people needs to submit your death certificate to the Risk Benefits Department in Cape Town.
  • The money will be paid out to the nominated person and must be used towards your funeral.
  • As the benefit is paid out in cash, it is vital that the people you choose are people you can trust.

If you want additional family funeral cover

Go onto Imbizo and search for the Momentum funeral scheme. You can then apply to have your family covered by a funeral policy. You will have to be included in the cover as well, but as funerals are expensive, the additional cover will not go amiss.

Your funeral assistance benefit

In the event of your death, or that of your immediate family member, the following scale of lump sum benefits will be paid to you, your beneficiaries or your deceased estate to help pay for your funeral:

Your deathR30 000
Your SpouseR30 000
Children aged 14 years and overR30 000
Children aged 6-13 yearsR15 000
Children aged 1-5 yearsR7 500
Still born to 11 monthsR3 750
  • The benefit is paid on a scaled basis and covers the employee and eligible immediate family members.
  • You must nominate two trusted people to receive this benefit. The money will be paid to one of the nominated people and must be used towards your funeral costs.

    • Complete all the details of your beneficiaries. Ensure that the information is correct.
    • You can nominate two people on the Funeral Benefit Nomination Form.
    • All nominees must be 18 years or older on the date that you complete and sign the form.
    • One of these two people needs to submit your death certificate to the Risk Benefits Department in Cape Town.
    • Ensure that your nominated beneficiaries are aware that you are providing their information to us for this purpose.
    • The money will be paid out to the nominated persons and must be used towards your funeral.
